New Students Registration

Please bring the following items to Registration:

1. Social Security Card (Not a Copy)

2. State Certified Birth Certificate

3. Immunization Record

4. Physical (done by a Tennessee Physician or Tennessee
    health-care provider.

6. Address & Phone Number of previous school

7. Copy of last year's report card, if possible.
